Charles Howenstine DDS explains that tooth erosion and tooth decay are often confused because both involve the loss of enamel, but they are caused by different mechanisms and require different responses. Understanding the distinction helps patients identify which risk factors apply to their situation and take appropriate steps.
Decay is caused by bacterial acids. Bacteria in the mouth metabolize sugars and produce acid as a byproduct. That acid attacks the enamel in localized areas, typically in the grooves of back teeth, between teeth, or along the gumline. Decay is a disease process involving the microbiome and is influenced by hygiene, diet, saliva, and fluoride.
Erosion is caused by direct acid exposure from non-bacterial sources. Common causes include frequent consumption of acidic beverages such as citrus juices, sodas, and sports drinks, as well as acid reflux or GERD, where stomach acid reaches the mouth. Erosion tends to affect tooth surfaces in a broader, more generalized pattern. The palatal surfaces of upper front teeth are commonly affected in patients with reflux because stomach acid pools in that area during episodes.
The clinical appearance is different as well. Eroded surfaces tend to look smooth and glossy rather than soft or discolored. Cupping, or saucer-shaped depressions on biting surfaces, is a characteristic sign of erosion.
Charles Howenstine DDS notes that arresting erosion requires identifying and reducing the acid source. For dietary erosion, this means reducing the frequency of acidic beverage consumption and rinsing with water after exposure. Drinking acidic beverages through a straw reduces contact with tooth surfaces. Brushing immediately after acid exposure should be avoided because the enamel is temporarily softened and more vulnerable to abrasion.
For patients with reflux, dental management works alongside medical management of the underlying condition. Fluoride helps strengthen enamel and improve resistance to acid attack. Once significant erosion has occurred, the surface loss is permanent and may eventually require restorative intervention.
